If you get BC disability assistance (PWD — Persons with Disabilities designation) and a serious, worsening health condition is causing you to waste away, you may get extra money each month for nutrition items and vitamins. ▸ Monthly amounts (rate table): • Extra nutrition items: up to $180 • Vitamins or minerals: up to $45 ▸ Who can get this? • You have the PWD designation and get disability assistance • A severe medical condition is making your health steadily worse • You have two or more of: malnutrition, being underweight, major weight loss, major muscle loss, major nerve damage, a failing vital organ, or a weakened immune system • Without these items your life would be in danger ▸ How to apply: 1. Ask the ministry for the MNS application form (HR2847): My Self Serve, 1-866-866-0800, or a ministry office. 2. Sign Part B. 3. Have your doctor, nurse practitioner, or registered dietitian fill out Part C. (Doctors can bill MSP for this.) 4. Return the form to your ministry office (Kelowna: 405 Highway 33 West; West Kelowna: 3685 Hoskins Rd).
