Call Home Health Central Intake to ask for home and community care for yourself or a family member. You can refer yourself. ▸ What you can get: • Nursing at home or in a community clinic (wound care, IV care, pain management, palliative care). • Occupational and physical therapy. • Home support with bathing, dressing and grooming. • Adult day programs and caregiver support, including respite. ▸ Who can use it: • Canadian citizens or permanent residents who need care after a hospital stay, care at home to stay out of hospital, or care for a life-limiting illness. • Some services also need you to be 19 or older and have lived in B.C. for 3 months. ▸ How to access: 1. Call 1-800-707-8550. 2. A health professional will assess what you need and what you qualify for. 3. Doctors and nurse practitioners can send a referral form instead. ▸ What to have ready: your Personal Health Number, your medications, your most recent tax return or notice of assessment, and your doctor's name and phone number. ▸ Cost: Some services are free (nursing, care management, occupational and physical therapy). Others have a fee based on your income.
