A Service Coordinator from the Peachland Wellness Centre (PWC) can come to your home, learn what you need, and make a personal service plan. The goal is to help you stay in your own home as long as possible. ▸ The plan may include: • Weekly or monthly visits from the Service Coordinator • PWC services like a Friendly Visitor/Caller, Transportation or Better at Home • Referrals to outside help, like Community Care, a pharmacist or a doctor ▸ Help for short-term problems (like after a hospital stay) or ongoing needs. ▸ Who it's for: all ages and residents of Peachland. Most people who call are 65 or older. ▸ How to access: • Call 250-767-0141. You can call for yourself, or a family member, health professional, friend or neighbour can call for you. • The Service Coordinator will contact you, assess your needs and make a plan. ▸ Hours: Monday–Friday 9:00 AM – 3:00 PM, by appointment ▸ Address: 4475 4th Street, Peachland, BC V0H 1X6 ▸ Not a replacement for medical care.
