Short-term specialized care (four to eight weeks) for older adults with complex health needs. A team looks at your whole health and makes a plan with you. ▸ Who can use this? • You need a referral from your family doctor, nurse practitioner or specialist. You cannot refer yourself. • Usually for people 65 or older (younger by exception) who are medically complex and can come to several appointments. • Concerns can include frailty, falls, memory or thinking changes, pain, weight loss, bladder or bowel problems, and medication concerns. ▸ The team may include A geriatrician, family doctors with a special interest in older adults, a mental health clinician, physiotherapist, occupational therapist, dietitian, pharmacist, nurse, social worker, speech-language pathologist and respiratory therapist. ▸ Where and when 505 Doyle Avenue, 2nd floor, Kelowna.
