Interior Health Home and Community Care helps people recover at home after a hospital stay. Home care nursing services include post-hospital care, wound care, IV care and self-care teaching. ▸ How it works A care manager works with you, your family and your doctor or nurse practitioner. They help plan your care, arrange services and support you through changes in care. ▸ How to get it • You or your family can call Home Health Central Intake: 1-800-707-8550. • Your doctor or nurse practitioner can send a referral form. An assessment decides what services you can get, for how long, and any costs. ▸ Not included House cleaning, laundry, grocery shopping and driving to appointments.
