CONNECT Lake Country — Brain Injury Rehabilitation Homes (Connect Communities): Fees apply, by phone, at 11438 Bottom Wood Lake Road, Lake Country, BC.
Categories: Disability, Health
CONNECT Lake Country offers rehabilitation in a home setting for adults living with brain injury and stroke. It has 42 private bedrooms in six condo-style homes. A team of coaches, occupational therapists, physiotherapists, recreation therapists and nurses helps each person work toward their own goals in real-life settings. This is called the Life Redesign Model. ▸ How do I get this? Call or email Janette Jackman, Service Access and Transitions Leader, at 604-534-0705. Referrals do not go through a health authority intake line. Families, lawyers, case managers, OTs and insurers can all start the conversation. ▸ Steps: 1. A first conversation to see if it's a good fit 2. A needs assessment and a cost proposal for your insurer or funder 3. A tour of the home (in person or online) 4. Rehabilitation in 3-month funding cycles 5. A planned move home or to a less dependent setting ▸ Good to know: The Lake Country homes are paid for through private funding arrangements, not health authority funding. CONNECT is accredited with Exemplary Standing by Accreditation Canada. Lake Country is about 30 minutes from Kelowna.
