If you are a WFN Member living with a long-term illness, you can meet monthly with the Community Care Nurse. ▸ Each visit includes: • Health checks • Time to talk about your health • Health education It is for illnesses like diabetes, high blood pressure, heart disease, asthma and arthritis (and others). ▸ Who can join? WFN Members only. A doctor needs to be involved for you to be admitted. ▸ How to book: call Community Services reception at 250-768-0227 ext. 2, Monday to Friday, 8:30 AM - 4:30 PM.
