Children and Youth With Special Needs - Transition Planning - MCFD

Helps families plan ahead when a child with special needs is turning 19 and childhood services will end. Starting at age 16, a worker can help with planning for adult services through Community Living BC, adult health care, applying for Persons with Disabilities status, and other future supports.
